Make An Appointment
When making an appointment please have your insurance/method of payment available. Also, if you need special assistance at your appointment, such as an interpreter, please note this when making your appointment.
When preparing for your appointment, please be sure to bring a copy of:
- Health insurance cards
- Photo ID
- Your current medications, including nutritional supplements
- Proof of income documents
If you are unable to make your appointment, please call the appropriate scheduling line to cancel or reschedule at least 24 hours in advance. Please refer to THCC’s No-Show Policy for more information.
